Steps to Log In to the Provider Portal

1. To log in to the Provider Portal go to https://secure.clearwaveinc.com
2. Enter your Username and Password in the appropriate fields and click Log in.  See Figure 1.
3. Select the Registration Location you will be working in from the drop-down list provided.  See Figure 2. 
4. Click Continue to complete the log in.
